Incident Reporting System for Active Duty Crew Members

Members, in a effort to streamline the incident reporting procedure and provider for a direct filing system of your reports we have developed this page with you in mind.
Now, whether at the station or at home you have the anonymity to file your report and know that it will be sent directly to the Executive Director.
Please note, that for vehicle maintenance request you should still continue to file the paper form located in the operations office and provide it to the duty supervisor for disposition.
Also, should you be involved in an accident with a unit and have a police report to file with your incident report, please provide that document to the duty supervisor for routing by placing the report in an envelope, writing executive director on the front and returning it to the supervisor or placing it in the chart box. Otherwise use the procedure listed below:

3. Then click on submit form in top right corner of page to send.
4. Your finished
You will be notified that it has been received and then when the disposition of the report is completed.
