Members, in a effort to streamline the exposure control reporting procedure and provider for a direct filing system of your reports we have developed this page with you in mind.

Your participation in this program is paramount to our surveillance of exposure protection plan.

The report you file will be directed to the management team for follow up.

Procedure:

3. Then click on submit form in top right corner of page to send.

4. Your finished

You will be notified that it has been received and then when the disposition of the report is completed.
